Staff Feedback Form – Director
Employees should feel free to speak to directors on an ongoing basis about any problems or issues, at any time of year. Formal evaluations will be done once each year, as a formal check-in.
The director feedback form below is just a guide: employees should give feedback on each of the four categories, but don’t need to answer each question on the list. These questions are just a guide for the type of information you might like to comment on.
This feedback will be shared with the departmental director and the executive director and will be part of the director’s formal annual performance review.
